CONSTRUCTION CREWS

1.0 BARGE SUPERINTENDENT/OFFSHORE CONSTRUCTION SUPERINTENDENT/OFFSHORE CONSTRUCTION MANAGER (OCM)/OFFSHORE INSTALLATION MANAGER (OIM) 

Answers to:

The Offshore Construction Superintendent reports to the Project Manager and provides verbal and written reports on the overall operational aspects of the vessel. He is the main sole responsible for ALL the offshore operations on the vessel down from the engine room to the bridge. The responsibility list below says it all.

All personnel and crew reports either directly or indirectly to the Offshore Construction Superintendent.

The Offshore Construction Superintendent liaises with the following personnel:

  • Project Manger
  • Operations Engineer
  • Offshore Installation Engineer on various technical aspects.
  • All other operating project personnel concerned with the project and the vessel.
  • Safety Coordinator
  • Client’s Representative
  • Vessel Captain and Marine Crew
  • Catering Contractor
  • Subcontractor’s representatives on the barge

Responsibilities:

Specific responsibilities of the Offshore Construction Superintendent include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Supervising the overall safe installation of the project specified and determined in the Project Manual & Procedures Liaising with offshore Operations and project management daily to ensure continuity of works and to report on progress.
  • Providing adequate and competent supervision of safe work practices and job instruction.
  • In the event of an emergency, by ensuring that all duties as detailed in the Emergency Procedures Manual are performed correctly.
  • Ensure all offshore Operations procedures are followed.
  • Ensuring that overall on-station operations are coordinated.
  • Approving requisitions for all necessary materials, equipment and supplies for the project and the vessel.
  • Responsible for reviewing and determining that the vessel and its associated spread can safely perform its function, relative to prevailing sea conditions.
  • Implementation of disciplinary procedures.
  • Conducting and participating in job safety analysis and safety meetings.
  • Establish proper lines of communications with the Company Site Representative to ensure a quality installation as stipulated in the project installation procedures.
  • Ensure regular lines of communication are adhered to by all supervisors to maximize efficiency, productivity and safety.
  • Conduct Monthly Crew Safety Talks.
  • Conduct Safety Stand downs

Specific

A) Safety and Life-Saving Appliances

  • Ensure safety and life-saving equipment possesses valid certification, is available in the required quantities, stowed and protected (where necessary) for immediate use:-

Safety helmets, protective clothing and equipment used where needed.

  • Life vests
  • Life rafts
  • Rescue equipment
  • Fire fighting equipment
  • Emergency distress equipment
  • H2S equipment

B) Safety Awareness

  • Ensure written instructions specifying practices to be observed to ensure safe operations are drawn to the attention of the people concerned and adhered to.
  • Ensure every person subject to his authority is made aware of the provisions of the HSE Policy & Company Policy which are relevant to him.
  • Be knowledgeable of the scope of the Company Health and Safety and Welfare Regulations in effect at the site and ensure that all relevant provisions are brought to the attention of every person on the installation.
  • Ensure timely reporting is made and a log is kept of all injuries, near misses, incidents, property and/or environmental damage.

C) Safety Training and Drills

  • Ensure crew members and visitors to the site have had the required safety training.
  • Ensure the necessary emergency drills are held at the required intervals including diving operations drills.
  • Ensure that a Muster List is displayed showing the station to which each person should go to in the event of an emergency and any duties to be carried out which are assigned to particular people.
  • Ensure that a muster is held regularly so that the ALL vessel crew are aware of the steps to be taken during emergency especially the new on signers or first timers

D) Medical / First Aid Resources

  • Ensure there is at least one (1) medically trained person onboard to take responsibility for medical treatment.
  • With the assistance of a medically trained person, the Offshore Construction Superintendent is responsible for the control of medicines and the organization of the sick Bay

E) Hazardous Areas and Work Permits

  • Ensure the control and administration of the work permit system through a responsible person which is normally monitored by the safety officer.
  • Ensure proper control is maintained to restrict exposure of personnel to ionizing radiation on the vessel or barge

F) General Maintenance and Housekeeping

  • Ensure all parts of the offshore site and construction equipment are maintained as to ensure the safety of the installation and the safety and health of the persons onboard.
  • Ensure that dangerous substances/materials are under the control of a responsible person and instructions relating to their storage and use are adhered to.
  • Ensure work places and access ways, including scaffolding and other temporary works, are safe and specific requirements are adhered to.
  • Ensure procedures/regulations concerning guarding and operation of dangerous machinery/equipment are complied with.
  • Ensure drinking water, provisions and accommodations are checked and are of the required quality.
  • Ensure that provisions are made for the correct disposal/removal of solid and liquid wastes at the site.
  • Ensure awareness of requirements and/or prohibition of asbestos and its handling.
  • Ensure that proper care is taken to limit the exposure of personnel to lead.

G) Operational Information

  • Ensure weather and sea conditions are systematically ascertained and reported.
  • Ensure an accurate record is maintained of the number of hours worked by each person at the site and a daily update of the record of all persons at the site is kept.
  • Ensure a daily record of all significant operations / activities and vessel Movements at the site is kept.
  • Ensure that all notifications are issued and/or received pursuant to any upcoming inclement weather.

H) Marine Operations

  • Control marine movements in relation to the site and, in particular, ensure all vessels are moored in accordance using approved procedures.
  • Ensure the transfer of personnel between vessels at the site is done in a safe and premeditated manner and accurate records are kept of personnel movements.
  • Ensure all marine vessels are operated within their load line limitations.

I) Helicopter Operations and Movements

  • Control helicopter operations and other movements in relation to the site and in particular ensure a competent Helicopter Landing Officer (HLO) is appointed to take responsibility for specific duties.

J) Lifting Operations

  • Ensure proper arrangements have been made to cover independent examination of lifting appliances and gear.
  • Ensure safe working loads are marked on lifting appliances, lifting capacities of gear and equipment are not exceeded and that any equipment reported unsafe is not used.
  • Ensure only experienced personnel operate lifting equipment and supervise lifting operations.

K) Diving Operations

  • Exercise overall responsibility for the proper organization of diving operations at the site.

L) Accident & Incident Investigations

  • Conduct, lead and control accident investigations in compliance with approved investigation procedures.

M) Close-out Action Follow-up

  • When required oversee, participate in and ensure all resulting close-out actions are follow-up for all safety inspections, auditing and investigations.

4.0 BARGE FOREMAN/DECK FOREMAN

The Barge Foreman reports to the offshore construction superintendent and is responsible for coordinating the work operations via the Deck crew.

Reporting to the Barge Foreman are Welder Foreman, Marine Captains and crew, Diving Supervisor, all Rigging, Scaffolding and Crane Operating personnel and Subcontractors for :-

The Barge Foreman liaises with the following personnel:-

Responsibilities

Specific responsibilities of the Barge Foreman include, but are not limited to, the following

  • Responsible for refusing unsafe work and for carrying our all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures, and ensuring all subordinates do also.
  • Wearing, inspecting, caring for and replacing required personal protective equipment, and ensuring all subordinates do likewise.
  • Coordinating deck work activities of the deck Crew as instructed by the Offshore Construction Superintendent.
  • Ensuring that all activities are undertaken in accordance with all Safety and Quality requirements.
  • Directing the Crane Operators in all their activities.
  • In the event of emergency, coordinating Emergency Team(s) as instructed by the Offshore Superintendent and as detailed in Emergency Procedures.
  • Ensuring appropriate ‘JSA’s have been developed for the work and using JSA and tool box meetings to eliminate unsafe acts and reduce or eliminate frequency and number of injuries.
  • Ensuring the house keeping of the above deck areas and rigger lockers are neat and tidy and free from unnecessary obstruction and hazard.
  • Ensuring accurate inventories are maintained for rigging, tools, scaffolding, etc.
  • Ensuring the rigging, scaffolding, portable equipment and tools, etc., are inspected before use and properly stored after use.

Any other duties as may, from time to time, be assigned by the Offshore Construction Superintendent.

5.0 SAFETY OFFICER

Answer to:

The Safety Officers on offshore vessels reports directly to Offshore Construction Superintendent.

Responsibilities:

Specific responsibilities of the Safety Officer include, but are not limited to directing efforts related to ensuring a safe work environment including the following:

  • Conduct safety and hygiene inspections, issue corrective action lists and follow-up on the close-out of these actions
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Chair and report on site safety meetings assign action and monitor their follow-up.
  • Conduct safety inspections at site continuously.
  • Advise anybody on board to stop all work should unsafe working conditions exist or persist.
  • Provide written instruction to correct unsafe work conditions.
  • Follow-up and report on safety induction and audit action lists.
  • Perform safety inspection briefing for all arrivals to the vessel or worksite.
  • Ensure all transport vessels assigned to the spread or site provide safety briefings to their passengers and audit these briefings.
  • Monitor subcontractors for safety equipment use and safe practices, and provide reports to the Offshore Construction Superintendent for non-compliance.
  • Monitor all site operations for proper equipment and work practices and provide recommendations for safety improvement.
  • Initiate, co-ordinate and record emergency drill, and make recommendations for the improvement of their effectiveness.
  • Investigate and report on all accidents at site and follow-up on all closeout actions to prevent reoccurrence.
  • Maintain all safety records at site.
  • Perform safety briefings, provide training and conduct safety reinforcement sessions at the site.
  • Monitor environmental issues at the site.

Any other tasks as may be assigned by the Offshore Construction Superintendent onshore Safety Manager.

6.0 BARGE DOCTOR

Answers to:

The Barge Doctor reports to the Offshore Construction Superintendent and is responsible for all medical and health matters on the barge/vessel.  In some cases, sometimes the vessel medic is not a qualified doctor but a medical assistance.

The Barge Doctor reports to the onshore Health, Safety and Environment Department.

The Barge Doctor liaises with the camp boss in matters of hygiene.

Responsibilities

Specific responsibilities of the Barge Doctor include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Providing medical treatment to any personnel on board who are sick or injured.
  • Making recommendations regarding the medivac of personnel and assist in their transport onshore as necessary.
  • Maintaining medical records of all treatment provided to sick or injured personnel.
  • Ordering medical supplies and ensuring that sufficient stocks are maintained.
  • Ensuring that a Drugs Log Book is maintained.
  • Overseeing any reports on condition of sanitation, food stores, food preparation and hygienic conditions of the vessel.
  • Assisting the Safety Officer on board in cases of accident investigation.
  • Monitor personnel who has declared their specific medication from time to time basis

7.0 MEDICAL TECHNICIAN/VESSEL MEDIC/OFFSHORE MEDIC

Answers to:

The Medical Technician reports to the Barge Superintendent or similar level and is responsible to provide pre-hospital care to accident victims and treating minor llnesses or injuries on the barge.

Responsibilities:

Specific responsibilities of the Medical Technician include, but are not limited to, the following :-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Inspects and maintains all emergency equipment on the barge/vessel in function readiness condition.
  • Monitors the general health of the crew and refer cases to company doctor whenever necessary.
  • Advises Offshore Construction Superintendent on health and medical matters.
  • Inventory medical supplies and equipment. Raise requisitions whenever necessary to ensure adequate medical supplies and equipment.
  • Ensures controlled drugs are securely stored in fixed steel cabinets, maintaining up-to-date register and conduct monthly inventory.
  • Conducts weekly health and sanitation inspection.
  • Monitors the validity of food handlers’ medical, stool examination and typhoid vaccinations.
  • Compile weekly and monthly health reports for management perusal and actions.
  • Ensures the cleanliness and functions of the clinic.

The Leaderman reports to the Barge Foreman and /or Offshore Construction Superintendent and is responsible to direct crew in the performance of tasks such as loading and unloading material barges.

The Leaderman liaises with the Riggers and Scaffolders

Responsibilities:

Specific responsibilities of the Leaderman include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work.
  • Following all company policies and procedures, and ensuring all subordinates do so.
  • Wearing, inspecting, caring for and replacing required personal protective equipment, and ensuring all subordinates do likewise.
  • Receive instructions from Barge Foreman and instructs crew in the performance of various tasks.
  • Inspects machines, equipment, incoming materials and completed tasks.
  • Performs the same tasks, when necessary, as the rigging crew.
  • Ensures work specifications are properly adhered to.
  • Ensures that tasks are carried out in a safe and proper manner.
  • Ensuring the house keeping of the above deck areas and rigger lockers are neat and tidy and free from unnecessary obstruction and hazard.
  • Ensuring the rigging, scaffolding, portable equipment and tools etc are inspected before use and properly stored after use.
  • Ensuring accurate inventories are maintained for riggings, tools, scaffolding etc.

Any other duties as may, from time to time, be assigned by the Offshore Vessel Superintendent.

10.0 RIGGER FOREMAN

Answers to:

The Rigger Foreman reports to the Barge Foreman and/or Offshore Construction Superintendent and is responsible in supervising project work operations via deck crew.

The Rigger Foreman liaises with the Rigger Leadermen, Riggers, Scaffolders and Crane Operators.

Responsibilities:

Specific responsibilities of the Rigger Foreman include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Supervise the daily tasks of the rigging crew.
  • Ensuring that all activities are undertaken in accordance with all safety and quality requirements.
  • Organize and distribution of above deck activities.
  • Loading and unloading of supply vessels.
  • Directing the crane operators in all their activities.
  • Continuously monitor rigging equipment being used and remove from service any found defective.
  • General housekeeping on the deck and rigger lockers.
  • Ensures only qualified scaffolders erect scaffolding.

11.0 RIGGER LEADERMAN

Answers to:

The Rigger Leaderman reports to the Rigger Foreman, Barge Foreman and/or Offshore Construction Superintendent and is responsible in assisting the Barge Foreman and Rigger Foreman in supervising project work operations via deck crew.

The Rigger Leaderman liaises with the Riggers, Scaffolders and Crane Operators.

Responsibilities

Specific responsibilities of the Rigger Leaderman include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Assist in supervising the daily tasks of the rigging crew.
  • Assist in ensuring that all activities are undertaken in accordance with all safety and quality requirements.
  • Distribution of above deck activities.
  • Assist in the loading and unloading of supply vessels.
  • Dogs deck cranes and big rig as requires.
  • Continuously monitor rigging equipment being used and remove from service any found defective.
  • General house keeping on the deck and rigger lockers.
  • Ensures only qualified scaffolders erect scaffolding.

12.0 RIGGER

Answers to:

The Rigger reports to the Rigger Foreman, Leaderman and Barge Foreman and is responsible in all aspects of deck activities.

Responsibilities

Specific responsibilities of the Rigger include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • To carry out general housekeeping.
  • All general rigging and de-rigging pertaining to barge activities.
  • Loading and offloading materials.
  • Mooring and un-mooring barges and boats alongside.
  • Any other activities as instructed by the Rigger Leaderman and Foreman.
  • Participate in the Emergency Response Team.
  • Continuously monitor rigging equipment being used and remove from service any found defective.
  • Participate in the project work on the barge i.e. grinding, assists with making of field joint etc.
  • Continually cleans the general deck areas to remove potential obstructions and return equipment and materials not in use to their designated storage areas.

The Derrick Crane/Big Rig Operator liaises with the Barge Foreman and Rigger Foreman.

Responsibilities:

Specific responsibilities of the Derrick Crane Operator include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Operates Derrick crane to lift and move heavy objects such as pipe, machinery and jackets.
  • Operates in response to signal either by hand or radio instructions by foreman.
  • To lift and position jacket into designated position.
  • To lift and install topside onto jacket.
  • Maintains responsibility for safe operation of derrick crane.
  • To report any malfunctions, wear or fatigue to Barge Foreman
  • To perform daily checklist of crane and complete checklist.

16.0 CRANE OPERATOR

Answers to:

The Crane Operator reports to the Offshore Construction Superintendent, Barge Foreman, Rigger Foreman and is responsible to operate gantry and Crawler cranes on barge.

Responsibilities

Specific responsibilities of the Crane Operator include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Operates gantry or crawler crane to lift and move heavy objects such as pipe, machinery and risers.
  • Operates in response to signal either by hand or radio instructions by Barge foreman, Leaderman and Rigger Foreman.
  • Maintains responsibility for safe operation of cranes.
  • To report any malfunctions, wear or fatigue to Barge Foreman.
  • To perform daily checklist of crane and complete checklist

Answers to:

The Anchor Foreman reports to the Barge Foreman and/or Offshore Construction Superintendent and is responsible for all anchor handling operations.

The Anchor Foreman liaises with the Anchor Hoist Operators.

Responsibilities

Specific responsibilities of the Anchor Foreman include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Direct anchor handling operations.
  • Assumes Leaderman responsibilities when not running anchors

21.0 BARGE/VESSEL WAREHOUSEMAN

Answers to:

The warehouseman reports to the Senior Warehouseman and Chief Engineer or similar level and is responsible to in the daily operation of barge warehouse.

Responsibilities

Specific responsibilities of the warehouseman include, but are not limited to, the following:-

  • Responsible for refusing unsafe work and for carrying out all work in accordance with the established safety standards and work practices.
  • Following all company policies and procedures.
  • Wearing, inspecting, caring for and replacing required personal protective equipment.
  • Responsible of warehouse inventory system.
  • Issue of material from warehouse.
  • Prepare material requisition.
  • Maintain material requisitions records.
  • Receiving and storing materials
